Please be aware is a specialist practice only accepting people experiencing homelessness. The staff there champion inclusion health and are also well versed in supporting people who may find keeping appointment times/ travelling to the surgery challenging. Most of the people who use this surgery have complex medical needs and struggle with the standard bureaucracy of most GP practices. I can't speak on if the practice offers shared care or will assist with monitoring bloods for DIY, but they do follow harm reduction approaches with substance use, so it would be likely for them to offer monitoring bloods.
If you're trans and experiencing homelessness this is probably an amazing resource.
